All admission and enrollment forms must be completed and enrollment fee and first tuition payment paid prior to your child’s first day of attendance.
An enrollment fee of $40 is due at the time of enrollment.
This fee is non-refundable.
A one-week, non-refundable tuition deposit is required for each child enrolled.
This deposit will be held without interest and will represent tuition payment for the last week of the child’s enrollment.
Based on the availability and openings, our facility admits children from 0 to 12 years of age.
Children are admitted without regard to race culture, sex, religion, national origin, or disability.
We do not discriminate on the basis of special needs as long as a safe, supportive environment can be provided.
The following documents are required to be completed and on file prior to attendance:
Registration Form - All required information must be supplied and a current emergency contact other then parents or guardian. In addition, an email address must be listed on the form. Any changes in information
must be updated immediately.
Contract - Contract must be on file with parent’s initials and signature.
Immunization Records - All immunization are required for the safety of your child, and others in our care.
Medical Release - All required information must be supplied to include current family provider and hospital of choice. The family handbook must be read thoroughly before the child's first day of attendance. Parents will be provided below, in writing, all information laid out in licensing standards.
The Parent Acknowledgement - Must be signed and turned in before the child can attend Playful Learners Preschool.
